This statement provides information about the practice and the treatment offered to assist you in choosing the treatment and the provider which best suits your needs. It is every client’s right to refuse or discontinue treatment at any time. Every client has the right to choose a practitioner and treatment modality that best suits their needs.

Collecting and holding information
​
The information is gathered as part of the assessment, diagnosis and treatment of the client’s condition, and is seen only by the treating therapist. The information is retained in order to document what happens during sessions, and enables the therapist to provide a relevant and informed therapeutic service. This information is stored on a web-based, secured and protected patient management system.
Access to client information
​
At any stage, you as a client are entitled to request access to the information about you kept on file, unless the relevant legislation provides otherwise. The therapist may discuss with you appropriate forms of access.
Confidentiality
​
All registered therapists and social workers are required to abide by guidelines of ethical conduct as set by the Australian Counselling Association (ACA) and Australian Association of Social Workers (AASW).
All personal information gathered by the clinician during the provision of the service will remain confidential and secure except where:

Information is subpoenaed by a court, or
-
Failure to disclose the information would place you or another person at serious and imminent risk, or
-
Your prior approval has been obtained to provide a written report or discuss the material with another person, professional or agency
Cancellation Policy

We certainly understand that issues and emergencies arise, however, we ask that you always try to notify us with as much notice as possible should you need to cancel or reschedule your appointment. We offer a text message 48 hours prior to ensure you have enough time to notify us of any issues.

In the event that you need to cancel your appointment with less than 24 hours notice, a fee will be added to your next payable invoice. This is not a punitive action, but a means to cover costs for holding your room and loss of an hour. Rooms are always booked for you in advance. Non-attendance is not only an issue for the practice, but also prevents others from accessing this appointment time.
